Job summary

Enhabit Home Health & Hospice is seeking a qualified Registered Nurse to serve as a nurse manager within our Bozeman, MT home health team. You will coordinate care, review patient information, and liaise with physicians, referral sources, patients and staff to ensure seamless care delivery.

The role requires a current RN license, at least two years in nursing and one year in home health, strong regulatory knowledge, and solid tech skills.

Qualifications

• Graduate of an approved school of professional nursing.
• Valid clinical nursing license per state requirements.
• Minimum two years of nursing experience in last 36 months.
• Minimum one year of home health experience.
• Knowledge of federal, state and local laws governing home care.
• CPR certification required.
• Valid state driver's license and automobile liability insurance.
• Basic tech skills and experience with mobile devices.
• Preferred: management experience and bilingual English/Spanish.
• In-office attendance is essential (except designated full-time remote roles).

Responsibilities

• Ensure the overall coordination of home health services provided to the patient is delivered according to acceptable standards of practice and company procedures.
• Review and approve patient information submitted by the licensed professional during a start of care, recertification, resumption of care, or evaluation visit.
• Facilitate the relationship between physicians, referral sources, patients, caregivers, and employees.
• Ensure scheduling of patients to field clinicians and communication with field staff, patients, physicians, referral sources, caregivers, and other service providers in order to maintain proper care coordination and continuity of care.
