Qualifications

• Must be a graduate of an approved school of professional nursing.
• Must be licensed in the state you practice.
• At least two years of nursing experience.
• One year experience in home health or hospice.
• Knowledge of the federal, state, and local laws and regulatory guidelines governing home care.
• Basic demonstrated technology skills, including operation of a mobile device.
• Management experience is preferred.

Responsibilities

• Coordinate and ensure the overall delivery of home health services according to standards and procedures.
• Review and approve patient information during start of care, recertification, resumption of care, or evaluation visit.
• Facilitate the relationship between physicians, referral sources, patients, caregivers, and staff.
