Job summary

Accounting Search seeks a Clinical Manager to oversee patient care services and clinical staff within a home health operations setting. You will coordinate patient care, supervise staff, and ensure care delivery meets safety and quality standards.

You will lead CHAP/survey readiness, drive QAPI initiatives, monitor patient satisfaction, and ensure compliance with agency, state, and federal requirements. Travel to multiple sites may be required.

Qualifications

• Graduate from an approved school of professional nursing and licensed to practice as a registered nurse in the state of operation.
• 5 yrs RN experience with 2 yrs supervisor experience in Home Health.
• Experience managing RN services in home, community, or clinic is preferred.
• Must possess and maintain valid CPR certification.
• Must be a licensed driver with insured vehicle and be willing to travel to all locations.

Responsibilities

• Oversee Operations: manage day-to-day clinical and administrative office operations and serve as primary contact for staff and patients.
• Ensure Regulatory Compliance: align care delivery with professional standards and state/federal regulations.
• Provide Leadership & Backup: act as leader in absence of Territory Administrator and cover PCM/CSS roles.
• Coordinate Patient Care: manage intake/referral and develop individualized care plans with interdisciplinary team.
• Drive Quality & Assessment: evaluate care quality, assess patient needs, and coordinate transfers/discharges.
• Manage OASIS & Discharge Data: review discharge data and oversee discharge forms.
• Lead Quality Improvement (QAPI): direct QAPI initiatives and track incidents with corrective actions.
• Maintain Survey Readiness & Analytics: ensure accreditation readiness and conduct audit reviews.
• Oversee Emergency Preparedness: lead EPP activities and post-activation reporting.
