Job summary

MGA Homecare is seeking a Clinical Case Manager to supervise home care personnel and ensure compliance with state and federal regulations. You will coordinate patient plans of care, mentor field staff, and maintain quality documentation under physician directions.

The role requires an active RN license (BSN preferred), 2+ years in home health or acute care, strong leadership, and excellent organizational skills. CPR and a valid driver’s license are required.

Qualifications

• Current state license as a Registered Nurse; BSN preferred.
• Minimum of two years of nursing experience in a Home Health or Acute setting preferred.
• Demonstrate leadership capabilities with strong supervisory and interpersonal skills.
• Detail oriented, flexible, good organizational and time management skills.
• Knowledge of cost management for clinical services.
• Current CPR card and health certificate as applicable.
• Current state driver’s license and automobile insurance.

Responsibilities

• Develops and implements the treatment plan for each patient under the direction of physician.
• Completes all clinical documentation in a timely manner, including admissions, recertifications, resumptions of care; ensures documentation meets agency policies and state regulations.
• Reviews clinical documentation to ensure it is complete and reflects skilled services for payer reimbursement.
• Interviews and evaluates potential clinical field staff to ensure competency for safe patient care.
• Participates in risk management by reporting incidents and injuries.
• Supports the mission and goals of MGA Homecare.
