Life Care Home Health in Las Vegas, NV is seeking a Clinical Manager to lead clinical teams and ensure high-quality, compliant home health services. You will supervise field staff, coordinate patient care plans, and collaborate with interdisciplinary teams and families to deliver patient-centered care.

The role requires RN licensure, a health-related bachelor's degree, and 2+ years in home health or community health, with supervisory experience and knowledge of Medicare CoPs.

Qualifications

• Current licensure as a Registered Nurse (RN).
• Bachelor’s degree in nursing or a health-related field preferred.
• Minimum of two years as a Community Health Nurse or RN in a certified home health agency.
• Healthcare management or supervisory experience preferred.
• Strong knowledge of home health regulations, licensure requirements, and Medicare CoPs.

Responsibilities

• Plan, coordinate, manage, and evaluate clinical teams to meet standards and regulations.
• Supervise field staff and promote cost-effective, quality-driven care.
• Oversee patient care plans, recertifications, discharge planning, continuity of care.
• Lead regular multidisciplinary case management conferences.
• Ensure accurate, compliant clinical documentation.
• Monitor staff performance and productivity with feedback and corrective action.
