Sun Recruiting, Inc. is seeking a Patient Care Manager (RN) to lead our home health team and elevate patient care across the community. This clinical leadership role requires guiding staff, ensuring regulatory compliance, and maintaining high-quality services while fostering a culture of safety and compassion.

The role focuses on supervising clinical staff, coordinating patient care, and driving continuous improvement to improve patient outcomes and staff development.

Qualificações

• Registered nurse with current licensure to practice in the practicing state.
• 5 yrs RN experience with previous experience in home care setting.
• 2 years management or supervisory experience in home health, preferred.
• Knowledge of Medicare Conditions of Participation and federal, state, and local regulatory requirements

Responsabilidades

• Provide clinical supervision to ensure patient care strictly aligns with professional standards, agency policies, and regulatory requirements.
• Plan, coordinate, and monitor patient care to ensure high-quality outcomes and consistent service delivery.
• Supervise, support, and coach clinical personnel to maintain care excellence and encourage ongoing staff development.
• Facilitate effective, transparent communication among patients, families, referral sources, and care team members.
• Drive quality improvement initiatives and meet departmental performance goals.
• Model professional leadership, service excellence, and core organizational values.
• Maintain rigorous compliance through required training, accurate documentation, and audit cooperation.
• Promote a safe work environment by implementing and enforcing workplace safety protocols
