Qualifications

• Current unencumbered license for the state of Ohio to practice as an RN is required.
• BSN preferred.
• Two years of supervision experience in a Medicare-certified home health agency is required.
• One year of experience in a Medicare-certified home health agency is required.
• Recent OASIS competency is required.
• Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet is required.
• Proficiency with electronic medical records is required; Homecare Homebase is preferred.

Responsibilities

• Lead the clinical home health team to ensure high-quality patient care and staff performance.
• Conduct supervised patient visits to assess competency and care delivery.
• Participate in on-call rotations and be available during operating hours for nursing visits.
• Oversee direct and indirect patient care services and ensure accurate evaluation of patient needs.
• Develop, implement and update individualized plans of care and assess effectiveness.
• Audit clinical documentation and ensure compliance with Medicare regulations.
• Coordinate referrals and manage the integrated EMR system to improve data quality.
