Job summary

Team Select Home Care in Phoenix is seeking a Clinical Educator to mentor, educate and evaluate clinicians, guiding new hires and ongoing staff training to ensure proper implementation of training and care processes.

The role reports to the Director of Nursing/Director of Patient Care Services and requires 3–5 years of clinical experience, including home health, with strong coaching and OASIS knowledge. Competitive salary and benefits align with a mission of quality patient care.

Qualifications

• Three to five years of clinical experience, with at least two years in home health.
• Solid understanding of all OASIS questions and interpretations per policy.
• Successful mentoring or coaching experience with new staff.

Responsibilities

• Mentor, educate and evaluate clinicians and caregivers.
• Conduct regular training for new hires and ongoing education for staff.
• Ensure care plans and compliance with state/federal rules.
• Support clinical leadership in patient care coordination and outcomes.
• Assist with performance improvement and staff documentation quality.
