Job summary

Home Health Agency - Arizona is seeking a Clinical Educator to mentor, educate, and evaluate clinicians and caregivers. You will conduct training for new hires and ongoing staff development, ensuring adherence to the Plan of Care and agency policies across field teams.

You will collaborate with the clinical leadership to meet outcomes, coordinate patient care, and support QAPI initiatives while providing direct patient care when needed.

Qualifications

• Minimum of three to five years of clinical experience.
• Excellent computer skills and familiarity with field software.
• Ability to coach, guide and empower orientee to achieve success.
• Knowledge of state and federal laws, rules, regulations and agency policies.

Responsibilities

• Mentor, educate, and evaluate clinicians and caregivers.
• Conduct regular training classes for new hire orientation and ongoing education.
• Assist clinical team in achieving outcomes, productivity, and care coordination goals.
• Provide direct patient care and case management as assigned per policy.
