Job summary

ProHealth Group seeks a Licensed Practical Nurse (LPN) or Registered Nurse (RN) to support home health services coordination for patients in their homes, following physician orders and maintaining high-quality care. Qualifications

• Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
• Three to five (3-5) years of experience in nursing, preferred.
• One (1) year experience in home health nursing, preferred.
• Acceptance of philosophy and goals of Agency.
• Ability to exercise initiative and independent judgment.
• Ability to work with individuals to enlist cooperation of many people to perform/achieve a common goal.

Responsibilities

• Adheres to Agency policies and procedures.
• Participates in development of standards for safe and effective service to patients/families.
• Assists in recruiting nursing personnel and orienting new staff.
• Coordinates interdisciplinary team services and staffing patterns.
• Maintains documentation and participates in quality improvement activities.
