ProHealth Home Health & Hospice in Daphne, AL seeks a Licensed Practical Nurse or Registered Nurse to support the Clinical Manager/Administrator in coordinating home health services per physician orders. The role emphasizes high-quality patient care, adherence to agency policies, and availability during operating hours.

The position requires nursing licensure, 3–5 years of nursing experience, with preference for home health exposure.

Competenze

• Graduate of an approved school of nursing and currently licensed in the state(s) where practicing.
• 3-5 years of nursing experience preferred.
• 1 year of home health nursing experience preferred.

• Participates in developing standards for safe, effective patient/family service.
• Consults with Clinical Manager/Administrator to determine staffing patterns.
• Assists in recruiting nursing personnel.
• Assists in orienting new patient care personnel.
• Plans and consults on staff needs.
• Implements policies meeting Medicare, Medicaid, and accrediting requirements.
• Supports policies and uses appropriate channels for changes.
• Coordinates interdisciplinary team services.
• Organizes nursing services for safe, therapeutic care.

• Participates in studies and administrative duties as assigned.

• Assists with implementing and evaluating patient care plans.
• Reviews patient records for policy compliance.
• Participates in staff meetings and education sessions.
• Ensures supplies and equipment are available.
• Performs nursing duties during staff shortages.
• Available for on-call duties as required.
• Assists with staffing visit and on-call assignments.
• Performs other duties as assigned.
• Coordinates referral, intake, and authorization under supervision.
• Other duties as assigned.
