Job summary

Boost Home Healthcare - North Shore is seeking a licensed Practical Nurse (LPN) to serve as Clinical Coordinator. You will manage referral intake, care coordination, and office operations, with occasional field support. The role emphasizes patient-centered care and an employee-centric culture.

Qualified candidates are an LPN in MA with at least 1 year of home health experience and current BLS. Proficiency in MS Office and EMR systems like KanTime, Axxess, WellSky, or HCHB is a plus.

Qualifications

• Education: Graduate of an approved school of practical nursing.
• Active LPN license in Massachusetts.
• Minimum 1 year of home health experience preferred.
• Current BLS certification.
• Tech-savvy: comfortable using Microsoft Office and electronic health records (EHR); experience with systems like KanTime, Axxess, WellSky, or HCHB is a plus.

Responsibilities

• Coordinate intake of new referrals, review clinical information, and assess appropriateness for home health services.
• Communicate with referral sources, patients, and families to facilitate admissions.
• Verify patient insurance eligibility and obtain authorizations for reimbursement.
• Collaborate with the RN Clinical Manager to start care and align visits with staff availability.
• Provide administrative support for scheduling, orders, and documentation.
• Serve as liaison between clinicians, patients, referral partners, and payers.
• Maintain EMR records and stay compliant with Medicare/Medicaid and agency rules.
• Assist with onboarding coordination, payroll support, and basic workflows as needed.
• Provide on-call nursing support as needed.
