Job summary

Elara Caring is seeking a Licensed Practical Nurse Clinical Coordinator to join our team and advance home health services. In this role, you will support clinical teams, coordinate patient care, and manage documentation and administrative tasks across the assigned branch.

Responsibilities include reviewing HCHB notes, obtaining physician orders, maintaining the At-Risk registry, assisting patients with scheduling and authorizations, and conducting field visits as needed.

Qualifications

• Current, unrestricted LPN license in the state of employment.
• Graduate of an accredited school of nursing.
• Minimum of 1 year of nursing experience in a clinical setting.
• Ability to lift, push, pull, and move up to 50 pounds regularly.
• Ability to work in an office setting and travel within the branch area.
• Valid driver's license or reliable transportation for travel between locations.
• Bilingual English/Spanish preferred.

Responsibilities

• Coordinate patient care with clinical teams and maintain workflows.
• Review and process Homecare Homebase (HCHB) notes and tasks.
• Enter physician orders for review and approval.
• Monitor the At-Risk patient registry to support quality care.
• Assist patients with scheduling, authorizations, and inquiries.
• Complete patient chart audits and maintain clinical documentation.
• Verify required Medicare notices and protect patient confidentiality.
• Collaborate with clinical staff and case managers for coordinated care.
• Provide direct patient care through field visits as needed.
