Job summary

Elara Caring is seeking a Licensed Practical Nurse Clinical Coordinator to support clinical teams, coordinate patient care, and participate in field visits. The role involves processing Homecare Homebase notes, entering physician orders for review, and maintaining the at-risk patient registry to ensure quality care.

Requirements include an active LPN license, nursing accreditation, and at least 1 year of clinical experience.

Qualifications

• Current, unrestricted LPN license in the state of employment.
• Graduate of an accredited school of nursing.
• Minimum of 1 year of nursing experience in a clinical setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Ability to work in an office setting and travel within the assigned branch coverage area as needed.
• Must possess a valid driver's license or reliable transportation to travel between assigned work locations.
• Bilingual English/Spanish is preferred.

Responsibilities

• Coordinate patient care with clinical teams and maintain efficient workflows.
• Review and process Homecare Homebase coordination notes and administrative tasks.
• Obtain and enter physician orders for Clinical Team Manager review and approval.
• Monitor the At-Risk patient registry to support quality patient care.
• Assist patients with scheduling, authorizations, and addressing questions or concerns.
• Complete patient chart audits and maintain accurate clinical documentation.
• Verify Medicare notices are provided to patients and protect confidentiality.
• Collaborate with clinical staff and case managers to ensure coordinated patient care.
• Provide direct patient care through field visits as needed.
