HomeCentris Healthcare, LLC is seeking a Client Relationship Manager to join our Harrisburg team. This role focuses on building and maintaining positive relationships with clients and families, coordinating onboarding, scheduling caregivers, and ensuring high-quality home health care services.

The CRM acts as the client’s primary contact, collaborates with caregivers, nurses, and administrative staff, and contributes to service planning and performance improvements.

Qualifications

• Two years of scheduling experience.
• Previous experience in client relations, customer service, or healthcare administration (preferred).
• Knowledge of home health care services and regulations preferred.
• Excellent communication and interpersonal skills.
• Strong organizational and problem-solving abilities.
• Ability to work independently and collaboratively in a fast-paced environment.
• Proficiency in Microsoft Office Suite and EHR systems.
• Bilingual candidates encouraged to apply.

Responsabilités

• Client Intake and Assessment: conduct initial assessments with clients and families to determine needs and goals.
• Client Onboarding: coordinate onboarding process for new clients including paperwork and service explanations.
• Client Communication: serve as primary contact for clients and families with updates on care plans and scheduling.
• Relationship Building: develop strong relationships with clients and families to ensure satisfaction.
• Care Coordination: collaborate with caregivers, nurses, and administrative staff for seamless care.
• Quality Assurance: monitor quality of care and gather feedback for improvements.
• Community Outreach: represent the agency at events to promote services and referrals.
• Documentation: maintain accurate records of client interactions and care plans.
