Job summary

Advocate Health in Wisconsin is seeking a registered nurse to manage complex patient caseloads across specialty areas. You will perform comprehensive assessments, plan individualized care in line with physician orders, and provide direct nursing care while updating plans as patient needs evolve.

You will communicate with physicians, families, and care teams to ensure coordinated treatment and timely documentation.

Qualifications

• Typically requires 3 years of experience in clinical nursing.
• RN license in the state of practice must be active.
• BLS for Healthcare Providers certification within 6 months, unless exempt by leadership.

Responsibilities

• Assumes responsibility for caseloads consisting of patients in specialty areas.
• Conducts initial assessment to obtain thorough physical, social and environmental data.
• Establishes the plan of care in accordance with physician orders and patient needs.
• Provides direct patient care and adjusts nursing care processes as needed; evaluates outcomes.
• Communicates with physicians to confirm and update patients' plan of care.
• Facilitates discharge planning and ensures continuity of care after discharge.
