Job summary

Aurora Health Care in Wisconsin is seeking a Registered Nurse to manage caseloads across specialty areas, perform initial assessments, and establish individualized plans of care aligned with physician orders.

The role involves direct patient care, ongoing assessment, and updating care plans. You will document thoroughly, communicate with the care team, and participate in discharge planning and quality improvement efforts.

Qualifications

• RN license required.
• BLS for Healthcare Providers within 6 months.
• Valid driver's license and access to a car.
• Associate's Degree in Nursing is required.

Responsibilities

• Assumes responsibility for caseloads consisting of patients in specialty areas.
• Conducts initial assessment to obtain thorough physical, social and environmental data.
• Establishes the plan of care in accordance with the physician orders and patient needs.
• Provides direct patient care and adjusts nursing care processes as needed, and evaluates outcomes within an appropriate time frame.
• Provides ongoing assessment of patient response to treatments and teaching, updating the care plan as indicated.
• Completes documentation including visit assessment, OASIS, 485 and verbal orders promptly and accurately.
• Communicates with physicians to confirm and update patients' plan of care and ensures ongoing communication with the health care team.
• Facilitates discharge planning and coordinates with patient/family, physician, social worker, and staff.
• Participates in chart audits and quality improvement activities to enhance service quality.
• Maintains accurate clinical and patient records per agency, state, and federal guidelines.
