Job summary

Arista Home Care is seeking an LHCSA Coordinator to manage day-to-day patient scheduling and service delivery. You will coordinate with patients, families, home health aides, clinical staff, managed care plans, and leadership to ensure coverage remains within plan of care and authorization periods.

The role requires strong organizational and communication skills, experience with EVV/scheduling systems, and the ability to multitask in a fast-paced environment.

Qualifications

• High school diploma or equivalent required; Associate’s degree preferred.
• Experience in home care coordination, scheduling or related healthcare setting preferred.
• Familiarity with LHCSA operations and MLTC/MCO service authorization processes preferred.
• Experience with EVV, scheduling, or home care management software preferred.
• Strong organizational and communication skills with ability to multitask in fast-paced environments.
• Proficiency in Microsoft Office and scheduling/referral management systems.

Responsibilities

• Coordinate and maintain patient schedules within authorized hours and plans of care.
• Match aides to patients based on services, needs, geography, and language.
• Respond promptly to call-outs and scheduling gaps; secure replacement coverage.
• Communicate schedule changes to patients, families, caregivers and internal teams.
• Monitor hours against delivered care; identify under-utilization and gaps for follow-up.
• Maintain accurate EVV records and visit documentation per procedures.
• Review service authorizations to ensure adherence to approvals and dates.
