Job summary

Arista Home Care seeks an ALF HHA Coordinator to serve as the onsite liaison at the assigned assisted living facility, coordinating intake, HHA onboarding, and cluster care scheduling with the clinical team.

You will maintain accurate records, ensure timely service delivery, and communicate changes to patients, families, HHAs, facility staff, and management in accordance with the plan of care and agency procedures.

Qualifications

• High school diploma or equivalent required; Active HHA certificate required.
• Experience in home care, assisted living, caregiver scheduling, or cluster care coordination preferred.

Responsibilities

• Serve as onsite liaison between Arista Home Care and the ALF, coordinating patient service needs and staffing updates.
• Complete and coordinate intake paperwork for new patients with signatures and documents.
• Track missing intake documents and escalate delays to meet service timelines.
• Coordinate new patient starts with Intake and clinical team, confirming approvals and HHA coverage.
• Facilitate onboarding of new HHAs, coordinating with HR for clearance before scheduling.
• Orient new HHAs to facility procedures, communication channels, and scheduling expectations.
• Create and maintain cluster care schedules based on authorized services and plan of care.
• Respond to call-outs and coverage gaps; arrange replacements as needed.
• Maintain accurate patient and HHA rosters; communicate updates to relevant departments.
• Communicate changes in patient condition and care concerns to clinical team and management.
• Coordinate with nursing and Compliance to schedule aide assessments and trainings.
• Maintain timely, accurate records in agency systems and provide updates for monitoring.
