Job summary

CenterWell Home Health is seeking a Patient Services Coordinator to schedule visits and maintain care coordination with field staff, patients, physicians, and office operations. The role supports day-to-day staff management and ensures timely communication across branches.

You will handle discharges, OASIS collections, and scheduling reports, contributing to efficient operations and high-quality patient care.

Qualifications

• Must possess excellent communication skills and ability to interact with a diverse group.
• Must have at least 1 year of home health experience.
• Packet review / QI experience is preferred.
• Coding certification is preferred.

Responsibilities

• Manages schedules for all patients and updates agent unavailability.
• Initiates infection control forms and HRD infection reports.
• Supports field staff during busy times and assists with case conferences.
• Generates scheduling reports and prepares weekly Agent Schedules.
• Ensures supervisory visits are scheduled and discharges are planned.
