LifeCircles PACE is hiring a Home Health Aide to provide compassionate, in-home care for older adults in Holland, MI. The role supports participants to remain safely in their homes while preserving dignity and independence.

Expect light housekeeping, mobility assistance, and vital-check duties within a patient-focused care plan. The position requires a CNA with reliable transportation, HS diploma or GED, and strong communication skills.

Qualifications

• CNA certification as per state requirements is required.
• Prior experience caring for older adults is preferred.
• Reliable transportation and a valid driver's license are required.
• High school diploma or GED is required.
• Excellent communication and observation skills are essential.

Responsabilités

• Provide hands-on in-home care including bathing, grooming, dressing and toileting.
• Maintain comfort by assisting with housekeeping tasks and daily living activities.
• Support mealtime and ensure proper hydration for overall health.
• Assist with mobility, transfers and positioning to maintain safety.
• Monitor vital signs and report changes to the IDT promptly.
• Keep living areas tidy and stocked with essentials for participant safety.
• Respond with kindness and professionalism to participant needs.
• Document and communicate pertinent changes to the care team.
• Protect confidentiality while delivering compassionate care.

What You'll Do In This Role:

LifeCircles PACE is looking for a dedicated and compassionate Home Health Aide (HHA) to join our PACE (Program of All-Inclusive Care for the Elderly) team. In this role, you will provide vital in-home care and support that empowers older adults to remain safely in their homes while maintaining their dignity and independence.

Key Responsibilities

• Provide Hands-On Care in Their Home – Assist with personal care (ADLS) like bathing, grooming, dressing, and toileting while following the participant's Life Plan.
• Maintain Comfort & Care – Support housekeeping tasks such as laundry, sweeping, and grocery shopping
• Support Mealtime– Help participants enjoy their meals and stay hydrated for good health.
• Keep Things Moving – Support mobility, including turning, positioning, and safe transfers.
• Perform Vital Checks – Measure and record vital signs, weight, and intake/output.
• Maintain Comfort & Care – Keep rooms tidy, cozy, and well-stocked with essentials.
• Provide Responsive Support – Respond with kindness and compassion.
• Monitor with Watchful Eyes – Report any changes in conditions to the interdisciplinary team (IDT) and leadership ASAP.
• Demonstrate Respect & Professionalism – Maintain confidentiality while delivering top-notch care.

Skills, Knowledge & Expertise
