Job summary

LifeCircles PACE in Muskegon, MI is seeking a dedicated Home Health Aide (CNA) to join the Participant Care team. This full-time role provides vital in-home care that helps older adults remain safe and independent in their own homes.

The schedule is Monday–Friday, 11:00am–7:30pm, with every other weekend and holiday rotation. Compensation ranges from $18.87 to $20.43 per hour, with benefits for full-time staff.

Qualifications

• Certified Nurse Aide (CNA) per state requirements, required.
• Experience with older adults preferred.
• Valid driver's license required.
• High school diploma or GED required.

Responsibilities

• Provide hands-on care in the home following the participant's Life Plan.
• Assist with activities of daily living (bathing, grooming, dressing, toileting).
• Support housekeeping tasks such as laundry, sweeping, and grocery shopping.
• Assist with meals and hydration to promote good health.
• Support mobility and safe transfers.
• Record vital signs, weight, and intake/output.
• Maintain rooms and supplies; ensure a comfortable, safe environment.
• Report changes in condition to the IDT and leadership promptly.
• Maintain confidentiality and professionalism.

What You'll Do In This Role:

LifeCircles PACE is looking for a dedicated and compassionate Home Health Aide (HHA) to join our PACE (Program of All Inclusive Care for the Elderly) team. In this role, you will provide vital in-home care and support that empowers older adults to remain safely in their homes while maintaining their dignity and independence.

Key Responsibilities

• Provide Hands-On Care in Their Home - Assist with personal care (ADLS) like bathing, grooming, dressing, and toileting while following the participant's Life Plan.
• Maintain Comfort & Care - Support housekeeping tasks such as laundry, sweeping, and grocery shopping
• Support Mealtime - Help participants enjoy their meals and stay hydrated for good health.
• Keep Things Moving - Support mobility, including turning, positioning, and safe transfers.
• Perform Vital Checks - Measure and record vital signs, weight, and intake/output.
• Maintain Comfort & Care - Keep rooms tidy, cozy, and well-stocked with essentials.
• Provide Responsive Support - Respond with kindness and compassion.
• Monitor with Watchful Eyes - Report any changes in conditions to the interdisciplinary team (IDT) and leadership ASAP.
• Demonstrate Respect & Professionalism - Maintain confidentiality while delivering top-notch care.
