Job summary

LifeCircles PACE in Muskegon, MI is hiring a Home Health Aide (HHA) for a full-time, 8:00 am–4:30 pm schedule. You will deliver in-home care as part of the Participant Care team, supporting older adults to live safely at home with dignity and independence.

Required CNA certification, HS diploma or GED, and a valid driver's license; prior elder care experience is preferred. Competitive benefits and mileage reimbursement are included.

Responsibilities

• Provide Hands-On Care in the home (ADLs) following the Life Plan.
• Maintain comfort and care through housekeeping tasks and assistance.
• Support mealtimes and hydration for patient health.
• Assist with mobility, turning, positioning and safe transfers.
• Measure and record vital signs, weight and intake/output.
• Keep rooms tidy and well-stocked with essentials.
• Provide responsive, compassionate support to participants.
• Report condition changes to the IDT and leadership promptly.
• Maintain confidentiality and professional conduct.

What You'll Do In This Role:

LifeCircles PACE is looking for a dedicated and compassionate Home Health Aide (HHA) to join our PACE (Program of All-Inclusive Care for the Elderly) team. In this role, you will provide vital in-home care and support that empowers older adults to remain safely in their homes while maintaining their dignity and independence.

Key Responsibilities

• Provide Hands-On Care in Their Home – Assist with personal care (ADLS) like bathing, grooming, dressing, and toileting while following the participant's Life Plan.
• Maintain Comfort & Care – Support housekeeping tasks such as laundry, sweeping, and grocery shopping
• Support Mealtime– Help participants enjoy their meals and stay hydrated for good health.
• Keep Things Moving – Support mobility, including turning, positioning, and safe transfers.
• Perform Vital Checks – Measure and record vital signs, weight, and intake/output.
• Maintain Comfort & Care – Keep rooms tidy, cozy, and well-stocked with essentials.
• Provide Responsive Support – Respond with kindness and compassion.
• Monitor with Watchful Eyes – Report any changes in conditions to the interdisciplinary team (IDT) and leadership ASAP.
• Demonstrate Respect & Professionalism – Maintain confidentiality while delivering top-notch care.

Skills, Knowledge & Expertise

• Certified Nurse Aide (CNA) per state requirements, required.
• Previous experience providing care to older adults, preferred.
• Reliable transportation and valid driver’s license, required.
• High school diploma or GED, required.
• Excellent communication and observation skills.
