LifeCircles PACE is seeking a dedicated Home Health Aide to join our team in Holland, MI. This full-time role involves in-home care to help older adults stay safely at home with dignity and independence.

Responsibilities include personal care, mobility support, vital checks, meal assistance, and ensuring a clean, comfortable living environment. CNA certification, a valid driver’s license, and excellent communication are required or preferred.

Qualifications

• Certified Nurse Aide (CNA) per state requirements, required.
• Previous experience providing care to older adults, preferred.
• Reliable transportation and valid driver’s license, required.
• High school diploma or GED, required.
• Excellent communication and observation skills.

Responsabilités

• Provide Hands-On Care in Their Home – Assist with personal care (ADLS) like bathing, grooming, dressing, and toileting while following the participant's Life Plan.
• Maintain Comfort & Care – Support housekeeping tasks such as laundry, sweeping, and grocery shopping
• Support Mealtime– Help participants enjoy their meals and stay hydrated for good health.
• Keep Things Moving – Support mobility, including turning, positioning, and safe transfers.
• Perform Vital Checks – Measure and record vital signs, weight, and intake/output.
• Maintain Comfort & Care – Keep rooms tidy, cozy, and well-stocked with essentials.
• Provide Responsive Support – Respond with kindness and compassion.
• Monitor with Watchful Eyes – Report any changes in conditions to the interdisciplinary team (IDT) and leadership ASAP.
• Demonstrate Respect & Professionalism – Maintain confidentiality while delivering top-notch care.

Schedule: 40 hours per week | Monday – Friday with Holiday Rotation | 8:00 am – 4:30 pm

What You'll Do In This Role:

LifeCircles PACE is looking for a dedicated and compassionate Home Health Aide (HHA) to join our PACE (Program of All-Inclusive Care for the Elderly) team. In this role, you will provide vital in-home care and support that empowers older adults to remain safely in their homes while maintaining their dignity and independence.

Key Responsibilities
