Job summary

Heartland Home Health, a Gentiva company, is seeking an Admissions RN to welcome patients into our home health program, ensuring a smooth, professional admission experience from start to finish.

You will conduct initial assessments, initiate the Plan of Care, document findings, and serve as liaison between referral sources, patients, families, and the care team—while upholding nursing standards and regulatory compliance.

Qualifications

• Active RN license in the state of employment (or eligibility to obtain).
• 1+ year of high acuity nursing experience.
• Current CPR certification.
• Valid driver’s license, automobile insurance, and reliable transportation.
• Strong organizational, communication, and clinical decision-making skills.
• Ability to provide care in home and community settings.

Responsibilities

• Conduct comprehensive initial assessments and evaluations to determine patient eligibility and care needs.
• Initiate the Plan of Care in accordance with physician orders, clinical standards, and regulatory guidelines.
• Document all patient assessments, clinical findings, and admission records accurately and promptly.
• Serve as a liaison between referral sources, patients, families, and the interdisciplinary team to ensure a smooth and supportive onboarding process.
• Maintain compliance with all applicable federal, state, and agency policies.
• Uphold professional nursing standards while delivering compassionate, hands-on patient care.
