Heartland Home Health is seeking a Full-Time Weekday Admissions RN to warmly welcome patients into our home health program and ensure a smooth admission process.

Responsibilities include initial assessments, Plan of Care initiation, accurate documentation, and liaison with referral sources to support onboarding. A valid RN license and CPR are required, with 1+ year of high-acuity nursing experience preferred.

Qualifications

• Active RN license in the state of employment or eligibility.
• 1+ year of high acuity nursing experience.
• Compassionate, flexible, patient-centered approach.
• Comfort in home and community settings.
• Strong organizational and clinical decision skills.

Responsibilities

• Conduct comprehensive initial assessments to determine eligibility and care needs.
• Initiate Plan of Care per physician orders and standards.
• Document assessments and admission records accurately and promptly.
• Coordinate with referrals, patients, families, and the care team.
• Maintain compliance with federal, state, and agency policies.
• Deliver hands-on nursing care with compassion.
