Job summary
Pathways Healthcare seeks a qualified Home Health Nurse (RN or LPN) in Massachusetts to perform patient assessments, administer medications, and develop individualized home-based care plans for rehabilitation and hospice patients.
The nurse will collaborate with physicians and interdisciplinary teams, provide caregiver support, supervise aides, and ensure compliance with healthcare regulations and BLS CPR standards. A valid MA license and reliable vehicle required.
Qualifications
- Active RN or LPN license in Massachusetts.
- Nursing degree from an accredited institution.
- Excellent bedside manners and communication skills.
- Working knowledge of healthcare laws and regulations.
- Maintains current BLS CPR certification.
Responsibilities
- Initiate preventive and rehabilitative nursing procedures.
- Administer medications and treatments as prescribed.
- Assess patients and families to determine home care needs.
- Develop and revise plans of care based on patient status.
- Collaborate with physicians and interdisciplinary teams on care plans.
- Address inquiries from patients, caregivers, and medical teams.
- Provide counseling and support to patients and caregivers.
- Supervise home health aides as required.
- Identify discharge planning needs and implement them.
