Pathways Healthcare in Lawrence, MA is seeking a dedicated Registered Nurse to join our home health and hospice team. You will provide patient-centered care, assess needs, administer treatments, and collaborate with physicians to build robust plans of care for home-based patients.

The role emphasizes high-quality bedside care, strong communication, and adherence to healthcare regulations. A valid RN license, 2+ years in home health preferred, and a flexible schedule are required for this

Qualifications

• Must hold an active Nursing license in the State where the position is based.
• Preferably 2+ years of nursing experience, with a focus on home health and geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.

Responsibilities

• Initiates appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Responsively address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
