Job summary

Pathways Healthcare is seeking an experienced registered nurse to join our home health team in Massachusetts. You will provide patient-centered in-home nursing care, implement and monitor plans of care, and collaborate with physicians to optimize outcomes.

Qualifications include a nursing degree, 2+ years of nursing experience (geriatric care preferred), EMR proficiency, and a valid driver’s license. We offer on-site support, flexible scheduling, and ongoing professional development.

Qualifications

• 2+ years of nursing experience, geriatrics preferred.
• New nurses welcome; strong training program available.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records (EMR).
• Excellent bedside manner and communication skills.
• Knowledge of healthcare laws and regulations.
• Maintain valid BLS CPR certification.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Assess and evaluate patient status and implement plan of care.
• Regularly re-evaluate patient and family needs.
• Develop and revise care plans with patient and family involvement.
• Administer medications and treatments as prescribed.
• Document clinical notes and monitor pain/symptom changes.
• Communicate with physicians regarding patient needs.
