Qualifications

Position Summary

The Home Health Admissions Coordinator models the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation, promoting the philosophy based on the 6 Pillars of Success. S/he upholds the Code of Ethical Conduct and fosters positive relationships within the company, with all departments, and external stakeholders. The coordinator manages referrals sent to the branches, oversees the process from intake to scheduling the admission visit, and notifies referral sources of non-admissions. Additionally, s/he coordinates completed home health referrals, including verifying orders, patient demographics, insurance info, F2F encounter notes, and physician verification for services.

Position Specific Responsibilities

  • Verifies all referral elements are present for an admission.
  • Coordinates referral management with the Growth team and Care Transition Coordinators (CTCs).
  • Maintains knowledge of program referral capacity issues.
  • Participates in quality improvement programs and daily stand-up meetings.
  • Applies policies and regulations, including HIPAA, Face-to-Face, and other certifying agencies.
  • Maintains confidentiality of information.
  • Develops and maintains relationships with team disciplines.
  • Utilizes best practices for pre-qualifying referrals.
  • Documents and routes service issues per policy.
  • Participates in training and interdisciplinary team meetings.
  • Prepares reports, documents non-admit referrals, and obtains necessary orders and documentation.
  • Provides information to patients/families and communicates referral updates to staff.
  • Ensures referral source information accuracy and captures required intake data.
  • Performs other duties as assigned.

Education and/or Experience

  • High school diploma or GED required.
  • Associate or Bachelor's degree preferred.
  • 2-3 years of related experience preferred; experience in insurance and Medicare/Medicaid billing is a plus.

Skills

  • Mathematical Skills: Basic arithmetic, rates, ratios, percentages.
  • Language Skills: Reading, analyzing, interpreting business and regulatory documents, and effective communication.
  • Other Skills: Proficient in English, effective communication with healthcare professionals, and embracing hospice philosophy.
