What You Need to Know

The Territory Patient Admission Coordinator is responsible for the timely acceptance or decline of referrals based on the agency's criteria. This position covers territories that may include one or more states and may change according to the company's needs. The Coordinator establishes communication with clients, families, and referral sources, partnering with staff, referral sources, and sales to ensure patient satisfaction and positive outcomes. They facilitate gathering all necessary information and documentation for referrals through follow-up with the agency, referral source, and client.

Qualifications

If you meet these qualifications, we want to hear from you!

• Licensed LVN or PT in your practicing state
• 3-7 years of experience in facility/physician relationships, with a good understanding of discharge processes (preferred)
• At least one year of experience in home care, hospice, or personal care services (preferred)
• Experience with Electronic Medical Records (EMR) (preferred)
• Experience with various healthcare payors like Medicare, Medicaid, and commercial payers (preferred)
