Adventist HealthCare is seeking an experienced Admission Registered Nurse for our Rockville Home Health team. This role focuses on comprehensive home-based patient assessment, care planning, and coordination with an interdisciplinary team to meet individualized goals.

You will document care, supervise Home Health Aides and LPNs, and support staff development while adhering to agency standards and quality initiatives in a non-exempt, 40-hour workweek.

Qualificações

• Graduation from an accredited nursing program.
• 2 years full-time experience in acute care.
• Current Maryland State nursing license.
• Current American Heart Association BLS certification.
• Valid Driver’s License and auto insurance as required.
• Certification in specialty once qualified.

Responsabilidades

• Assessment, planning, intervention & evaluation of patient care in the home to achieve patient goals.
• Documentation of patient care and outcomes (electronic if applicable).
• Delegation and supervision of tasks Home Health Aides and LPNs.
• Serves as a support to new staff or students.
• Participates in quality initiatives.
• Collaborates with the interdisciplinary care team in patient care planning.
• Pursues professional development and growth.
• Meets standards set by agency demonstrating flexibility, commitment and adaptability.
