Job summary

Pruitthealth Corporation in Georgia is seeking a Licensed Care Manager Home First. You will screen referrals, coordinate admissions, and work with providers to ensure seamless service delivery in patients' homes.

The role requires an active LPN license, a valid driver’s license, and at least five years of nursing experience (hospice or SOURCE care preferred). Travel within the service area will be required, with on-call rotations as needed.

Qualifications

• Current State License as a Licensed Practical Nurse (LPN).
• Must have a valid driver’s license and automobile insurance liability.
• Graduate of a state approved accredited nursing school.
• Certified in Basic Cardiopulmonary Resuscitation.
• Minimum of five (5) years of nursing experience; Hospice or SOURCE care preferred.

Responsibilities

• Screens appropriate referrals and coordinates admission process.
• Coordinates services with providers and billing reconciliation.
• Provides outreach to community referral sources.
• Participates in weekly team meetings and presents all new admits/annual assessments/hospitalizations/discharges.
• Completes quarterly home visits and PCP visits with all requirements.
• Tracks hospital/ER visits and coordinates with hospitals.
• Arranges transportation and handles audits when required.
• Trains new hires and participates in on-call rotation as designated.
