Job summary

PruittHealth in Georgia seeks a Licensed Care Manager Home First to help coordinate admissions, assess needs, and connect patients with appropriate in-home services. You will screen referrals, manage documentation, and collaborate with medical providers to ensure timely and quality care.

The role requires a current LPN license, a valid driver’s license, CPR certification, and at least five years of nursing experience, with hospice or SOURCE care preferred.

Qualifications

• Current State License as a Licensed Practical Nurse (LPN).
• Valid driver’s license and automobile insurance liability.
• Graduate of a state approved accredited nursing school.
• Certified in Basic Cardiopulmonary Resuscitation (CPR).
• Minimum of five (5) years of nursing experience; Hospice or SOURCE care preferred.

Responsibilities

• Screens appropriate referrals and coordinates admission process.
• Coordinates services with providers and handles billing reconciliation.
• Outreach to community referral sources and attends team meetings weekly.
• Completes admission paperwork, in-home assessments, and quarterly visits.
• Manages variances, plans corrective actions, and follows up to resolution.
• Coordinates with outpatient therapy centers, home health agencies, and rehab.
• Uses own vehicle for travel; maintains mileage/time sheets for reimbursement.
• Participates in on-call rotation and communicates with regulators as required.

Education

Licensed Practical Nurse (LPN) license

Graduate of state approved accredited nursing school

Certified in Basic Cardiopulmonary Resuscitation
