Job summary

U.S. Renal Care is seeking a Home Therapy Registered Nurse to deliver patient training and ongoing support for home dialysis modalities. You will perform home visits, assess environments, and coordinate care with physicians and the care team.

The role requires an active RN license, dialysis experience, CPR certification, and strong communication skills in English. You will participate in on-call rotations and contribute to continuous quality improvement efforts.

Qualifications

• Current RN license in applicable state.
• 12 months of clinical nursing experience plus 3 months home modality experience; Texas requires 6 months in home modality.
• Experience with dialysis is preferred.
• CPR certification required prior to patient care.
• Clear ability to communicate in English (verbal and written).
• Complete Home Therapy Nurse Orientation and annual competency.
• Proficiency with USRC clinical applications within 90 days.
• Meet state practice requirements and any additional clearances if applicable.

Responsibilities

• Provide patient training and ongoing support for home dialysis.
• Conduct home visits to assess the patient’s home environment and care needs.
• Communicate assessment results to physician and care team.
• Document nursing services in the EMR and ensure continuity of care.
• Participate in infection control monitoring and reporting.
• Review Emergency Preparedness procedures with home patients.
• Coordinate supplies and orders to avoid delays and penalties.
• Track patient outcomes and support QA activities.
• Obtain routine lab tests and communicate urgent results.
• Administer medications as ordered and participate in on-call rotations.
