Job summary

U.S. Renal Care is seeking a Home Therapy Registered Nurse to provide patient training and ongoing support for home dialysis modalities. This role focuses on delivering high-quality care and excellent customer service during internal and external interactions.

Responsibilities include home visits, patient assessment, documentation in EMR, infection control, emergency preparedness, inventory management, and collaboration with physicians and care teams.

Qualifications

• Current RN license in applicable state.
• 12 months of clinical nursing experience and 3 months in home dialysis modality, or per state requirements.
• CPR certification required prior to patient care assignment.
• Confirmation of ability to distinguish all primary colors.
• Demonstrated working knowledge of the English language in speaking and writing.
• Basic computer skills; proficiency in USRC clinical applications within 90 days of hire.
• Must meet state practice requirements and any background clearances if applicable.

Responsibilities

• Provide patient training and ongoing support for home dialysis modality.
• Conduct home visits to assess patient home environment and care needs.
• Document nursing services in the EMR with status and interventions.
• Communicate results to physician and care team as appropriate.
• Participate in infection control monitoring and emergency preparedness.
• Ensure inventory and placement of orders for supplies to avoid penalties.
• Assist in quality improvement activities through data collection and analysis.
• Perform on-call nursing services, nights and weekends as needed.
• Be flexible with staffing locations and hours to meet patient needs.
• Maintain regular attendance.
