Job summary

U.S. Renal Care is seeking a Home Therapy Registered Nurse to train patients and provide ongoing support for home hemodialysis or peritoneal dialysis.

The role includes conducting home visits, documenting care, and coordinating with physicians to ensure high-quality patient outcomes. Responsibilities span patient education, clinical assessments, infection control, and on-call coverage, with flexibility to work across locations and ensure timely supply management and quality improvement

Qualifications

• RN license in applicable state (current and in good standing)
• 12 months of clinical nursing experience
• 3 months experience in the home dialysis modality (or more stringent state requirements)
• CPR certification required before patient care
• Ability to distinguish primary colors
• Completion of Home Therapy Nurse Orientation and annual skills competency
• English language proficiency (verbal and written)
• Basic computer skills in USRC clinical applications

Responsibilities

• Provide patient training and ongoing support for home dialysis modalities
• Perform home visits to assess patient home environment and care needs
• Document nursing services in EMR including training and interactions
• Communicate patient assessments and laboratory results to physicians and team
• Coordinate infection control monitoring and report findings
• Assist with emergency preparedness and equipment procedures
• Ensure supply inventory and timely orders to avoid penalties
• Monitor and improve patient outcomes through data review and QA activities
• Perform on-call nursing duties nights and weekends as assigned
• Maintain regular attendance and flexibility across locations
