Job summary

U.S. Renal Care is seeking a Home Therapy Registered Nurse to train patients for home dialysis and provide ongoing care. The role requires adherence to federal and state regulations, clinical excellence, and strong patient service.

Home visits, documentation, and collaboration with physicians are essential. The position includes on-call responsibilities, flexible home-based care locations, and commitment to quality outcomes.

Qualifications

• Current RN license in applicable state.
• 12 months of clinical nursing experience; 3 months in the home dialysis modality (Texas requires 6 months).
• Prior nursing care experience with dialysis patients preferred.
• CPR certification required before patient care assignment.
• Ability to distinguish primary colors.
• Must complete the Home Therapy Nurse Orientation and maintain competency.
• Proficiency in English (verbal and written).
• Basic computer skills; proficiency in USRC clinical applications within 90 days.
• Must meet state practice requirements and any related clearances.

Responsibilities

• Provide patient training and ongoing support for home dialysis.
• Conduct home visits to assess patient home environment and performance.
• Document nursing services in the EMR with accuracy and completeness.
• Communicate patient assessment results to physicians and team.
• Assist with infection control monitoring and reporting.
• Explain Emergency Preparedness procedures to home patients.
• Ensure patient supply inventory and timely orders to avoid penalties.
• Track program and patient outcomes to drive quality improvement.
• Obtain routine and non-routine lab tests and relay critical results.
• Complete monthly medication profile reviews and administer meds as ordered.
• Perform on-call nursing duties including nights and weekends as needed.
• Maintain flexible staffing locations and hours for patient needs.
• Attend to regular and reliable attendance as required.
• Coordinate home evaluation with technical services for facility readiness.
• Ensure water sampling and reporting per policy and product requirements.
