Job summary

Fresenius Medical Care is seeking an entry-level registered nurse for Home Therapies CAP 1 to provide coordinated home dialysis care and participate in a multidisciplinary team. Responsibilities include assessing patient responses, documenting plans, educating patients and families, and ensuring safe delivery of dialysis therapy at home.

The role involves collaboration with the Interdisciplinary Team, administration of prescribed medications, and participation in CAP progression and on-call

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful completion of CPR BLS Certification
• Must meet the practice requirements in the state in which he or she is employed.

Responsibilities

• Provide and coordinate clinically competent care for assigned patients per Fresenius protocols.
• Assess and manage patients’ response to home dialysis training and treatment.
• Document in the medical record and communicate with physicians as needed.
• Educate patients and family about learning needs.
• Direct and provide safe delivery of dialysis therapy with home care partners.
• Administer medications as prescribed with justification.
• Participate in safety, staff training and quality improvement.
• Ensure proper lab collection and processing; reschedule missed labs.
• Assist in transplant and treatment modality education.

Education

Graduate of an accredited School of Nursing
