Job summary

Fresenius Medical Care seeks a PCT II HT to join the home therapies team in Las Vegas. You will support home dialysis training, collect vitals, and coordinate with HT RNs and staff to ensure safe, compliant patient care.

Responsibilities include equipment setup, supply management, and documentation across home dialysis workflows, under supervision and in accordance with state and federal regulations.

Responsibilities

• Reinforces home hemodialysis (HHD) and/or peritoneal dialysis (PD) training.
• Identifies and welcomes patients, obtains vital signs, weights, and gathers information for intake forms.
• Builds and maintains communication with patients, care partners, HT RN, and staff.
• Obtains treatment records and reviews for completeness; reports missing/abnormal data.
• Assists with electronic home treatment/medication reconciliation and reports data to HT RN.
• May assist HT RNs with home visits.
• Provides home support as applicable.
• Prepares supplies for lab testing and ships specimens.
• Assists with tracking and shipping transplant-related specimens and education.
• Completes opening/closing chores and documents accordingly.
• Disinfects dialysis machines, chairs, tables, and equipment between patient visits.
• Prepares exam/training rooms and stocks rooms with supplies.
• Manages supplies to maximize use and minimize waste.
• Schedules patient appointments and communicates reminders.
• Supports patient registration and connected health use.
• Assists with supplies/inventory ordering and management.
• Maintains logs and tracking.
• Performs other clerical duties as assigned.
• Reinforces HHD training.
• Sets up, tears down, programs, tests, and disinfects the HHD machine.
• Troubleshoots HHD machinery/tools and connectivity issues.
• Prepares dialysate for use.
• Prepares dialysis supplies for treatment per orders.
• Evaluates dialysis access and documents findings; reports abnormal data.
• Initiates HHD treatment and records vitals and machine data.
• Performs safety checks during treatment as CMS/policy dictates.
• Reports patient status changes to HT RN; documents in chart.
• Provides interventions during interdialytic events; documents as directed.
• Obtains pre-treatment vitals and documents.
• Ensures hemostasis of access before leaving area.
• Collects post-treatment vitals and reports to HT RN.
• Assists with water and dialysate testing as directed.
