Job summary

Qualifications

• Minimum two years of patient care experience, preferably in hospitals or related settings.
• Must hold valid PCT certification or CMS-recognized state certification.
• High school diploma or equivalent required.

Responsibilities

• Assist in home dialysis training and equipment setup under RN supervision.
• Record vital signs, weights, and collect required clinical information.
• Maintain clean and safe home environment and supplies for home therapy.
• Prepare laboratory samples and assist with specimen transport.
• Disinfect dialysis equipment between patient visits.
• Schedule appointments and coordinate with patients and care partners.
• Support patient registration and use of connected health services.
• Participate in audits and quality improvement activities.
