Job summary

Fresenius Medical Care in Virginia is seeking a PCT II HT to join the home dialysis team under RN supervision. You will assist in setup, training, and ongoing patient care in the home therapy program, and help maintain a safe, clean environment following policies and regulations.

Responsibilities include reinforcing HHD/PD training, collecting vitals, coordinating with the HT RN, preparing supplies, and supporting patient registration and connected health, with adherence to CMS and CQI goals.

Qualifications

• High School diploma or G.E.D. is required.
• CPR certification may be required or preferred for patient care roles.
• Experience in hospital or clinical patient care is preferred.

Responsibilities

• Reinforces home hemodialysis (HHD) and/or peritoneal dialysis (PD) training as permitted by law.
• Identifies and welcomes patients, obtains vital signs, weights, and information for intake.
• Builds communication with patients, HT RN, and care staff; reports missing data to HT RN.
• Assists with preparing supplies, documentation, and lab specimen handling as directed.
