Job summary

Fresenius Medical Care is seeking a Patient Care Technician II in the Home Therapies program. Under direct supervision of the HT RN, you will reinforce HHD/PD training, assist with equipment setup and maintenance, and help prepare rooms and supplies for patient visits.

The role includes collecting vitals, documenting data, and coordinating with care partners to ensure safe, effective home dialysis therapy. Qualifications include a high school diploma or GED, and a current PCT CMS certification.

Qualifications

• High School diploma or GED required.
• Valid PCT national certification or state CMS certification required.
• CPR certification may be required for continued employment.
• Two years of patient care experience preferred.

Responsibilities

• Reinforces home hemodialysis (HHD) and peritoneal dialysis (PD) training.
• Assist with setup, care and maintenance of home dialysis equipment.
• Obtain vital signs, weigh patients and complete intake documentation.
• Support supplies management, scheduling and patient documentation.
• Collaborate with HT RN and care team during visits and shift huddles.
