Job summary

Fresenius Medical Care is seeking a patient care technician in our Home Therapies program. You will assist in setup and maintenance of home dialysis equipment, reinforce training for HHD/PD, and support patient education under the supervision of an HT RN.

Travel between facilities may be required as part of patient care coordination. The role emphasizes accurate data collection, collaboration with the HT RN, and adherence to CMS/state guidelines to ensure safe and effective home dialysis therapy

Qualifications

• Experience in dialysis patient care preferred.
• Ability to obtain vital signs and maintain accurate records.
• Strong communication with patients, families, and health care team.
• Knowledge of infection control practices in a clinical setting.

Responsibilities

• Reinforces home hemodialysis and PD training as permitted by law.
• Assists with setup and maintenance of home dialysis equipment.
• Tracks patient vitals, weights, and treatment data.
• Responsible for preparing supplies and maintaining clean training areas.
• Coordinates scheduling and patient communication.
• Assists with medical records and audit processes as directed.
• Maintains logs and helps with inventory and procurement as needed.
• Performs other clerical duties as assigned.
