Job summary

Fresenius Medical Care seeks a PCT II HT to support home dialysis therapy under RN supervision. You will assist in setting up equipment, maintaining a safe home environment, and coordinating training for patients and care partners.

You will reinforce HHD/PD training, collect vitals, and liaise with HT RN and staff to ensure quality care and smooth operations in the home therapy program.

Qualifications

• High School diploma or G.E.D. required.
• PCT national or state certification required.
• Two years of patient care experience preferred.
• CPR certification required.

Responsibilities

• Reinforces home hemodialysis (HHD) and/or PD training as permitted by law.
• Identifies and welcomes patients, obtains vital signs and necessary information.
• Maintains communication with HT RN, staff and other team members.
• Obtains treatment records and reports missing or abnormal data.
• Assists with home treatment data reconciliation and orders.
• Sets up/tears down HHD equipment and disinfects after visits.
• Prepares exam/training rooms and stocks supplies.
• Schedules appointments and coordinates patient reminders.
• Participates in record keeping, audits and patient education.

Education
