Job summary

Fresenius Medical Care is seeking a PCT III HT to join the home therapies team. Under the supervision of an HT RN, you will reinforce home hemodialysis and peritoneal dialysis training, assist with patient intake, vital signs, and data collection, and support the care team in delivering safe home dialysis services.

Responsibilities include preparing treatment rooms, handling supplies, coordinating with the HT RN, and participating in CQI activities to improve patient outcomes within the facility

Qualifications

• Five years of patient care experience in a hospital setting or related facility preferred.
• Must meet CMS/state licensure requirements for PCTs.
• Commitment to organization culture and patient care standards.

Responsibilities

• Reinforces HHD/PD training as permitted by law.
• Gather vitals and information for intake forms.
• Maintain communication with HT RN and care team.
• Assist with collection of specimens and lab testing.
• Prepare exam/training rooms and restock supplies.
• Support registration and use of connected health.
• Contribute to CQI and staff training programs.
• Perform clerical duties as assigned.
