Responsibilities

• Assist clients with basic living skills and daily personal tasks.
• Provide transportation to activities and appointments when required.
• Maintain documentation such as Behavior Reports, Incident Reports, and Monthly Attendance Logs per Medicaid guidelines.
• Communicate with staff regarding client needs and progress toward the care plan.
• Assist in Plan of Care development and monitoring for health, safety and security.
