Job summary

Rehab Without Walls Neuro Rehabilitation seeks an experienced Occupational Therapist to deliver functional rehabilitation in home, school, work, and community settings. You will assess needs, create treatment plans, and guide clients through real-life activities, from kitchen tasks to grocery shopping and workplace tasks.

You will work autonomously within a collaborative therapy team, conducting re-assessments, discharge planning, and coordinating with families and other disciplines.

Qualifications

• Bachelor’s degree in Occupational Therapy from an accredited program.
• Current, unrestricted OT license in the practicing state.
• Current CPR Certification.
• Minimum of one year’s OT work experience.
• Knowledge of rehabilitation techniques for complex neurological diagnoses preferred.
• Effective verbal and written communication.
• Ability to lift 50 pounds; patient handling up to 100 pounds with safety precautions.
• Duties require fine motor skills, visual acuity, and walking/standing for extended periods.
• Moving, lifting, or transferring patients with safety precautions.
• Health screen or exam may be required prior to assignment and periodically thereafter.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly re-assess effectiveness of treatment plans, attend staffing meetings and family conferences.
• Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources.
• Communicate patient’s needs and progress to the treatment team, physician, patient and family members.
• Communicates with other disciplines to ensure collaboration, coordination of care and enhance patient outcomes.
