Job summary

Purpose Home Health seeks a dedicated scheduling coordinator to support home-based care services in Indiana. The role focuses on fielding patient inquiries, coordinating daily schedules for home health aides, and logging visits to ensure timely and compliant care delivery.

Qualifications

• Experience coordinating home care or healthcare scheduling.
• Strong phone and interpersonal communication skills.
• Ability to log and monitor patient and aide activity in a system.

Responsibilities

• Field new patient/client inquiries over the phone in a knowledgeable manner for CICOA.
• Coordinate home health aides' schedules on a daily basis.
• Monitor, mediate, and log both patient/client and home health aides in the system.
• Follow up on home health assignments and patient/client services.
• Ensure optimal utilization of CICOA, WAIVER, VA, and Prior Authorization hours.
• Participate in on-call rotation - serving as support for home health aides and residents.
