Job summary

Assisted Home Health & Hospice in Covina is seeking a Home Care Specialist/Sales to join the Private Duty Department. The role focuses on intake coordination, consulting with families, and driving referrals for in-home care services.

A base hourly rate plus commission rewards performance and client growth. The ideal candidate has home care/private duty experience, strong organizational and communication skills, and a track record of meeting sales targets.

Qualifications

• Must have home care/private duty experience, including intake coordination.
• Strong communication and relationship-building skills with clients, families, and referral sources.
• Proven ability to meet monthly sales goals and collaborate with care teams.

Responsibilities

• Screen and facilitate referrals promptly, coordinating with HUB referrals, calls, or emails.
• Follow up on referrals: set appointments, mail information, and document activity in records.
• Meet with families to start of care, conduct home safety evaluations, sign consents, and develop care plans.
• Coordinate with Program Manager and Branch Manager to ensure safe utilization of caregiver services.
• Promote positive relationships between clients, families, staff, and payors; act as liaison.
• Develop community relationships and identify needs of organiza­tions and hospitals.
• Identify suitable clients with home health and hospice services and ensure follow-up support.
